What is considered a dental emergency?

Dental emergencies can either be non-critical or critical (true dental emergency).

Critical or True Dental Emergency

It involves moderate to severe pain that needs immediate attention. A few examples are:

Knocked out or broken tooth or teeth as a result of an accident
Severe constant pain
Large swelling in face or gums
Severe pain on biting
Uncontrolled bleeding after a dental surgery or tooth extraction

Non-critical Dental Emergency

These are mostly inconvenient situations that do not harm the individual and can be treated the next day. A few examples are:

Broken denture or other dental appliance
Loose orthodontic bracket
Small chips on teeth that are either very slightly sensitive or completely asymptomatic
Fillings fallen out where the tooth is asymptomatic
Broken porcelain on crowns

Should I go to the ER for a dental emergency?

It depends on the nature of the emergency. If you are having a severe swelling and do not have access to a dentist, you should go to the ER to control the swelling. If you have a broken tooth or a painful tooth, the ER will most likely not be able to solve your problem. Hospitals lack dentists to treat routine dental emergencies. They only treat cases that are truly a health emergency. They may use temporary measures to relieve pain until you are able to see your dentist the following day. It is always better to try contacting your dentist prior going to the ER.

I knocked out a tooth. What should I do?

With a proper action, the tooth that has been knocked out of its socket can be successfully saved. In such event, carefully pick up the fallen tooth by grabbing the crown. Do not touch the root or you will injure the cells lining the root surface. If the tooth has dirt on it, gently rinse it with water ONLY and do not scrub it. You have a very limited time to make a decision about how to store the tooth and visit a dentist.

Your options are:

Place the tooth back into the socket: This is the best option. Re-position the tooth back into its socket and hold it in place with your fingers. This allows oxygen rich blood to nourish the cells on the tooth root.
Save-A-Tooth Kit: The 2nd best option would be to place it in “Save-A-Tooth” solution; however, most of us do not have this type of kit handy at the time of an accident. You only have very limited time to handle the knocked out tooth. Going to the store to purchase this type of kit may not be an option unless you are near a pharmacy. It is important to know that the longer the tooth stays outside the mouth, the worse the prognosis. Anything past 30 minutes has a poor prognosis.
Place it in your mouth or milk: Your last resort would be to place the tooth inside your mouth by your cheek, or place it in milk until you can see a dentist.

Regardless of how you handle the tooth, the longer it takes you to see a dentist, the less likely it will be to save your tooth. Your dentist will be able to clean the tooth, reposition it back into the socket, and splint it for maximum stability.

What is the best over the counter pain medication for a severe toothache?

Advil, Motrin, or Aleve work well against dental pain due to their anti-inflammatory action. Only take the recommended dosage. All pain medications have a ceiling effect, meaning that past a certain dosage the medication will not provide you with any additional pain relief. Excessive dosage is quite toxic to your liver.

My dental implant fell out. What should I do?

This involves 2 scenarios:

Only the implant crown fell out because the cement was debonded, the screw anchoring the crown to the implant became loose, or the screw broke in half. There is not much you can do except calling your dentist. It can wait until the following day. Usually, it will be a simple matter of re-attaching your crown to the implant.
The implant itself failed and the entire implant body fell out. Take a look and if you see a cylindrical shaped metal body resembling a big screw with deep threads, then your implant has certainly failed and fallen out. Typically, the area is not treated right away; instead left alone until the gum fully heals prior to placing another implant. Call your dentist or visit a dental office the next day if you do not have access to one on the same day.
I have bleeding gums that won’t stop bleeding. What can I do?

This can be an issue after extractions. Ideally you need to contact your dentist or possibly go to the ER if you cannot get the bleeding under control. Follow the instructions given to you after your treatment. Applying constant pressure on the bleeding site using a clean piece of or placing a damp tea bag over the site while applying pressure should help with the bleeding as well.

What should you do if a dental crown fractures?

This is not a true dental emergency and you should be OK until you can see your dentist. The only way a broken crown can be fully restored to its original shape is by replacing it. Depending on the type of the crown and extend of fracture, sometimes they can be smoothed and left untreated.

Porcelain crown: If you have a crown that is entirely made of porcelain, once it fractures the only option would be a new crown. If it is a slight chip then it can be smoothed but the majority of these crowns exhibit a catastrophic fracture and need to be replaced.

Porcelain fused to metal crown: Unlike a porcelain crown, these types of crown have an inner metal layer that can never fracture, thus, no matter how many times the porcelain chips; the metal layer will always protect the underlying tooth structure. If you have a minor porcelain chip, it can be smoothed and left untreated. If it is a large chip and renders your tooth without any biting contact with the opposing tooth, it is highly recommended to replace the crown before it starts drifting.

What should I do if my veneer falls out?

As long as it is not fractured and has an acceptable seal, it can be cleaned and re-cemented back onto your tooth. Most common reasons as to why veneers fall out are:

Chronic excessive force from biting or clenching (can be re-bonded)
Failure of cement layer as a result of leakage (can be re-bonded)
Presence of caries which weakens the tooth and cement layer (will require a new veneer)
Can a tooth abscess harm you?

Do not take teeth abscesses lightly. If left untreated, it can truly become a serious health issue. Infections may appear small but they can double or triple in size overnight. The oral cavity is highly vascularized and it allows the infection to spread quite easily. That is why dentists are always worried about dental infections. Lower jaw abscesses can easily spread toward the back of the throat and severely restrict your breathing to a point that it becomes life threatening. Upper teeth abscesses can easily spread to your sinus, eyes, and brain.

You may think that antibiotics will take care of the infection but keep in mind that they need 24-48 hours to start taking effect during which time the swelling can quadruple and cause serious health issues.

Why is a toothache worse at night?

When you lie down at night, the blood pressure increases very slightly in the head and neck region. An unhealthy tooth is so sensitive to pressure changes that the slightest pressure fluctuation will result in additional pain. This is also the reason why you could experience toothache during flights. Place an extra pillow under your head or sleep sitting up to minimize the pain until you can see your dentist.

Will rinsing with salt water alleviate dental pain?

Almost all dental pain is internal, meaning they originate from inside the jaw bone or inside the tooth. Rinsing with mouthwash or salt water will not have any effect. If you have inflamed gums, the rinse could be beneficial.

What should I do with loose orthodontic wire/brackets?

Brackets can become debonded and feel uncomfortable by pinching your lip. If you have orthodontic wax, cover it until you can see your dentist the following day. Short term loose wires or brackets will not have any negative impact on your orthodontic treatment.

I swallowed my crown/filling, what now?

As far as the tooth is concerned, as long as you are not in constant severe pain, you will be OK to wait until the next day. In most instances, swallowing a crown or filling is not an issue as these will pass through your digestive tract. You should see your dentist regardless to determine why the crown had fallen out. Most likely there is decay that destabilized the crown or filling.

“Very good dentist. I had a crown break, he had me come right in for an exam and then gave me a couple of options. Explained very clearly the pros and cons of each and the price. Staff was very friendly also. Would definitely recommend.”
